Investigation Summary
On September 8, 2010, Employee #1 was performing maintenance from a portable roll around ladder. He was replacing component of an open end spinning machine, frame number 8. He lost his balance, fell from the ladder, and struck his head on the concert floor. Employee #1 was hospitalized and later died.
