Investigation Summary
Employee #1 was working in an area by himself, the only other coworker in the area was away from the work areas and did not see what occurred. Employee #1's head injury had indication that his head may had been crushed in between the stacker, and the support brace, but blood or other material could not be seen at the location of the accident since the scene had been cleaned and all or any evidence removed.
