Investigation Summary
Two employees were cutting a hole in a corrugated metal roof for a ventilation system. One employee was using a power saw to cut the metal and Employee #1 was holding the piece of metal to keep it from falling to the floor below. When the cut was completed, Employee #1 holding the sheet metal fell through the open hole. Employee #1 died.
