Employee Dies after Falling from Basket on Forklift

On January 27, 2011, an employee was standing inside a parts basket lifted up by a forklift. The employee leaned over the side of the parts basket, and then the basket and employee fell to the concrete floor. The fall was approximately 20 feet. The employee died as a result of his injuries. The parts basket was not secured to the Toyota forklift.
