Investigation Summary

Investigation Nr: 200542025
Event: 07/22/2010
Two Employees Die in Explosion

Employee #1 and Emplyee #2 died when molten zinc leaked from the tray column and into the firing chamber. The molten zinc vaporized and expanded in the firing chamber and combusted with the available oxygen. This expansion and combustion was significant enough to blow out all four explosion panels. This allowed the introduction of a significant amount of oxygen from the outside air which caused the vaporized zinc to combust and result in a fire ball.
