Employee Catches Foot in Conveyor and Sprains Ankle

Employee #1 was clearing a jam along a conveyor, when he decided to ride the conveyor while it was on. Employee #1's right foot got caught in a space between where two adjoining conveyors connect. Employee #1 sustained a sprained ankle. The conveyor had just 38 inches of vertical clearance between the top of the conveyor and the bottom of a 24-inch-deep bar joist above. There was an additional 24 to 25.5 inches of vertical clearance to the ceiling, for a total height of 62 to 63.5 inches. Investigation of the matter found that this employee was not even supposed to be on top the conveyor. Maintenance mechanics are the only employees allowed atop the conveyors during which time the conveyors are locked out and tagged out, and the maintenance mechanics are using personal fall arrest systems.
