Investigation Summary
At approximately 3:15 p.m. on May 12, 2010, Employee #1 was working on the rooftop to install sheathing and was not utilizing fall protection. Employee #1 had accessed the roof from the open attic rafters. The roof was a reverse gabled type, the roof pitch and measured 25 ft from the eave to the ground. Employee #1 had worked on the roof for approximately five minutes before falling. Employee #1 was life flighted to Shock Trauma and suffered a fractured seventh vertebrae. He underwent surgery was placed in a medically induced coma.
