Investigation Summary

Investigation Nr: 200054021
Event: 07/18/2011
Employee's Finger Is Amputated by Conveyor

At 10:45 p.m. on July 18, 2011, Employee #1 was working the second shift, in the packaging area of a pork processing facility in Omaha, NE. Employee #1 was loading a box onto the conveyor, when his finger went between the approximately 1-inch by 1.5-inch opening in the plastic links of the conveyor and was pinched against a plastic guide piece. The movement of the links against the guide piece created a scissor action that removed the tip of Employee #1's right middle finger and nail. Employee #1 was transported to Bergan Mercy Medical Center, where his wound was closed. After the injury, the employer installed a guard over the area where the employees placed the boxes to prevent additional injuries.
