Investigation Summary
At approximately 10:00 p.m. on February 15, 2011, an employee was working as a picker for the company where his duties had him picking steel materials off of racks and placing onto a forklift. A coworker was a forklift operator who operated a Raymond side loading forklift. The employee and a coworker worked as a team on the third shift picking and moving steel materials from the storage racks to the Braner tables for sorting and shipping. The coworker inspected the forklift and got the directions from the supervisor on their jobs for the night. At approximately 12:25 a.m., the employee and the coworker were working in aisle number 14 and were moving to aisle number 3 to make their next pick. The employee walked over to aisle number 3 by cutting through the openings located every 20 feet along each aisle row until he arrived at the gap between sections D and E of row number 4 where he was waiting for the coworker to bring the forklift over. At approximately 12:28 a.m., the coworker was driving the Raymond side loader south down the aisle between number 3 and number 4. He stated he did not see where the employee was standing until he realized his forklift had struck one of the trays the employer used to store the steel materials that were sticking out into the aisle. The impact damaged the battery box on the forklift causing a battery terminal to disconnect. The result of the loss of power was the emergency kill switch turning off the forklift, brakes failing and the forklift coasting to a stop. The other result of the impact was the tray was pushed five feet south, striking the employee and pinning him between the rack from section D and the stacks of material in section E. The employee struck his head and suffered major head trauma and massive blood loss. The coworker ran for help and to find the assistant supervisor. The management official on duty immediately called emergency medical services. The ambulance arrived at approximately 12:39 a.m., and the paramedics pronounced the employee dead from injuries shortly after their arrival.
