Investigation Summary
At 12:00 p.m. on November 5, 2010, Employee #1 was using a table saw that was unguarded to rip some window trim. While the saw was running, a stray dog bit Employee #1 on the leg, causing Employee #1 to make contact with the unguarded, rotating blade, resulting in a partial amputation of Employee #1's right thumb.
