Investigation Summary
Employee #1 was assisting a coworker move boxes of aluminum ductwork from the ground level to a mezzanine in the facility using a forklift, which did not have a guardrail system in place. Employee #1 and a coworker had moved two other pallets of boxes to the mezzanine previously. Employee #1 told the the coworker, that he was clear, and subsequently, the coworker backed the forklift out to get the next pallet. When the coworker looked back to move the forklift after getting the next pallet, he noticed Employee #1 on the ground.
