Investigation Summary

Investigation Nr: 201345352
Event: 05/27/2010
Employee Falls And Strikes Head On Pavement And Later Dies

On May 27, 2010, Employee #1 was attempting to cover a Travis trailer, which was pulled by a 2000 Kenworth cab and loaded with sand, by manually turning the crank handle of a Shur-lok tarp. The crank extension separated from the Splined U-Joint as result of the wire lock pin not being in place. Employee #1 fell backwards and struck his head on the asphalt street. The foreman of Employee #1, on an unrelated task, drove to the accident scene which was located outside of the company office, and proceeded to render medical assistance, contacted office personnel, and alerted the Crawford County KS EMS. The EMS responded and transported Employee #1 to the Mount Carmel Medical Center in Pittsburg, KS. Employee #1 was subsequently transferred to St. Lukes Hospital where he succumbed to his injuries.
