Investigation Summary
On June 10, 2010, Employee #1 was working with a crew placing a safety sign at the entrance to the oil rig location, when the crane contacted an overhead high voltage wire. Employee #1 was on the ground and had a chain in his hand that was attached to the crane and voltage passed through him to ground, and he was fatally injured. The crane operator was able to jump from crane cab and attempted to pull him from the chain but was thrown against a nearby container by the high voltage. The crane operator suffered injury and was released from the hospital the following day.
