Employee Is Killed in Fall from Roof

On October 30, 2009, Employee #1 and a coworker were replacing the shingles on a single family home. They were working on a roof with a 4 to12 pitch and an eave to ground height of approximately 18 ft. The rake to ground height varied because the ground was not level, but it was approximately 26 ft at a high point. They were laying out a string line to lay the next course of shingles. Employee #1 walked too close to the edge of the roof along the rake side and he fell approximately 22 ft onto the concrete steps. Employee #1 was killed. The employees were not using fall protection equipment.
