Investigation Summary
On November 18, 2009, Employee #1, of Integrated Industries Corporation, was assigned to hang a banner 40 ft tall on top of stacked inter-model containers. Employee #1, Employee #2 and Employee #3 (the Yard Manager) climbed on top of another separate container, which was being elevated by an empty container and hoisted by a crane operated by Employee #4. The employees rode the elevated container to the top of the stacked containers. When they went to descend from the stacked containers, they had to leap onto the elevated one. Employee #1 leapt first. When he hit the container, he landed on an end that gave way. However, the crane operator could not control it. The end swung down, tearing the opposite side support from the crane. Employee #1 fell to the ground and suffered unspecified injuries. He was hospitalized for treatment.
