Employee Crushed by Forklift

Employee #1 was found lying in front of an end load bucket, which was used to lift the rear end of a fork truck to repair a broken axel. It appears that Employee #1 used wooden blocks to hold up a rear axel, instead of a jack stands or an hydraulic jack, and the forklift crushed Employee #1 while he was under it. A Corner's preliminary report determined, that blunt force trauma to the chest with broken ribs, was the cause of death.
