Investigation Summary
Employee #1 was called upon to work on a temperature agitator with the motor placed on top of the agitator itself. A probe is used to mix the contents of a rail car and hold the temperature steady. The safety latch on the hook of the crane was pushed up and out, according to an eye witness, after the acident was discovered. Employee #1 suffered shortness of breath, and was taken to a hospital, and later flown to Chicago, where he died a few hours later.
