Employee Is Struck and Killed by Forklift

On May 18, 2010, an employee, an air purification systems technician, was returning from his break to his work site. He was walking on the shoulder of a dirt road. He was wearing hearing protection. A forklift, a Gehl DL-6L telescoping rough-terrain forklift, was being driven on the dirt road, and approached the employee from the rear. They were both going in the same direction. The operator of the forklift had an obstructed view to his right side, where the employee was, due to the lowered boom on the forklift. The forklift operator did not sound his horn as he approached the employee. As the forklift was about to pass the employee, it was reported that the employee stepped in front of the forklift and was struck and crushed by the forklift. The employee was taken to a local hospital, but he was pronounced dead upon arrival. His cause of death was from multiple blunt injuries which included multiple fractures, contusions and abrasions.
