Employee Amputates Finger While Removing Jam from Machine

At approximately 8:22 a.m. on January 13, 2010, Employee #1 was working in a tortilla manufacturing facility, where he was removing a dough ball that had become stuck in the cone of a prepress. The prepress consists of cylinders that lower into the cones and press the dough onto a conveyor belt. The area is guarded by an access door that measures about 83 inches high. Employee #1 was assigned as an assistant to a coworker and was not instructed to operate the prepress machine. As he was carrying bags of flour, he noticed that dough had become stuck in the cones of the prepress machine. He then got a ladder and reached into the unguarded opening above the access door and down into the cones to clean out the excess dough that was jamming up in the obstructed cone. The prepress cylinders activated and Employee #1's right hand came into contact with the moving parts. Employee #1 was taken to Methodist Hospital of Sacramento in Sacramento, California, where he was hospitalized and treated for the amputation of his right middle finger. The incident investigation cited the employer for the failure to guard the openings around the prepress section of the tortilla production lines, the failure to maintain continued effectiveness of hazardous energy control procedures, the lack of training in hazardous energy control procedures, and the failure to implement hazardous energy control procedures during the cleaning and removing the jam from the machine.
