Investigation Summary
At approximately 2:00 p.m. on November 8, 2010, Employee #1, an English-speaking male, was working at a company that reconditioned wood pallets. Employee #1 was operating a board trim saw to cut runners for new pallets. The board he was cutting slipped and caused his hand to contact the saw blade. Apparently, one or more of Employee #1's fingers were amputated, and he was hospitalized. The accident occurred indoors and no one witnessed it.
