Investigation Summary
At aproximately 11:00 a.m. on August 4, 2010, Employee #1 was fatally struck by a fallen interior masonry block wall during the construction of a school building. Employee #1 was attempting to pick up tools near the free standing block wall due to an impending storm approaching the site. As high winds prevailed, the unsupported block wall failed at the base, landing on Employee #1, crushing him underneath. No apparent wall bracing was in place at the time of the incident.
