Investigation Summary
At approximately 8:30 p.m. on July 25, 2010, Employee #1, a security guard of Guardsmark, LLC, was performing normal security checks using a Cushman powered industrial vehicle (Model Number 898459-8820). The vehicle had been modified after its manufacture to include a cab structure over the seating area, a tool storage compartment on one side of the vehicle's flat bed, an extension to the flatbed, and a second row seat. In addition, the vehicle was in use at a time when the tires were grossly under inflated. For some reason, during the operation of the vehicle, it tipped to its side. Employee #1 became pinned under the vehicle's cab frame, killing him due to crushing and asphyxiating injuries.
