Investigation Summary
On May 21, 2010, Employee #1 of L&W Engineering Company was injured when using a 20 ton overhead gantry crane to lift press dies off of a transfer table and metal plate. When Employee #1 lifted the dies, the dies did not immediately release from the metal plate. The metal plate was not secured to the table, and the plate rolled off of the table and onto the floor. The plate struck his right foot, causing a partial foot amputation. The accident occurred indoors; therefore weather was not a factor. There were no witnesses to the accident.
