Employee Crushed by Boiler

Employee #1 was engaged in cutting an 8,000 lb boiler in sections with a cutting torch. Employee #1 was performing this task from the inside of the boiler. A part of the section being cut, fell off, allowing for the remaining section, to flip over onto its bottom landing on Employee #1's lower back. A coworker removed the object and drove the Employee #1 to the hospital where he was pronounced dead.
