Employee Is Killed When Truck Tire Explodes

On October 20, 2009, Employee #1 was working with a paving crew laying asphalt. They were using a paver, shuttle buggy, and supplying dump trucks. Employee #1 normally worked on the dirt crew had been assigned that morning to monitor engagement of the shuttle buggy's truck hooks to the right rear tire of the dump truck. He was also to deflect asphalt from spilling on to the area to be paved and shoveling of spilled asphalt. An end dump truck backed into the unloading area guided by a coworker on the left side. The truck did not quite line up and the dump truck was realigned. Employee #1 was seen to be apparently kicking at the right hook during this time. The truck was backed in again and the hooks reengaged. The truck started dumping into the buggy's front hopper and was nearly done. A loud bang was heard. The loud noise was the dump trucks right outside tire rupturing when the right hook disengaged from the tire rim. The hook roller had gone into the tire side wall causing the 100 pounds per square inch of air to be released. Employee #1 was believed to be facing the tire, perhaps downhill. He was blown 10 ft away from the truck tire causing blunt force injuries to his right side. He was also peppered with small tire debris. Employee #1 was killed.
