Investigation Summary

Investigation Nr: 202422283
Event: 01/26/2010
Employee Is Killed When Run over by Tractor Trailer

At approximately 11:20 a.m. on January 26, 2010, Employee #1, a purchasing manager, was talking on the cell phone in a blind spot between the two trailers at a cement manufacturer. Another worker was driving an over-the-road truck for a transportation company, and was stopped on a plant road waiting to deliver a load of gypsum. When the truck ahead of him finished unloading and circled back towards him, he started to pull forward. The driver of the other truck radioed for him to stop, when he saw Employee #1 standing between the truck's trailers. Employee #1 was run over by the driver's side front wheels of the rear trailer. He fractured his head and upper torso. The driver of the over-the-road truck was unaware of Employee #1's presence.
