Employee Replacing Light Fixture Is Electrocuted

Employee #1 was replacing existing light fixtures with new light fixtures. Employee #1 was using a Genie boom lift Model Number Z-60/34, Serial Number 66259 and RSC Number 647546 to access the lights that were approximately 40-ft to 50-ft in the air. The stem of an old light fixture extended above the lift basket approximately 48 in., and made contact with the existing fixture that Employee #1 was going to unplug and replace. The stem got stuck in the existing light and when Employee #1 tried to remove the stem it came in contact with the element of the bulb resulting in an electrocution. The voltage at the plug was 277-volt with an internal transformer in the light fixture bringing voltage up to approximately 400-volts to 600-volts at the bulb. Employee #1 was killed.
