Investigation Summary
At approximately 9:45 a.m. on March 19, 2013, Employee #1 and a crew were dismantling a scaffold. As they dismantled the scaffold, a plank struck Employee #1, causing him to lose his balance and fall. He fell approximately 11 feet, striking the ground and sustaining head and face trauma. Employee #1 was transported to a medical center, where he underwent treatment, but succumbed to his injuries and died at the hospital on March 28, 2013.
