Investigation Summary
At 9:00 p.m. on August 1, 2012, Employee #1, a truck driver, suffered head trauma due to being run over by a truck. Employee #1 was pronounced dead at the scene. The facility involved in the accident is a third party logistics facility. Employee #1 had been working there for an unknown period of time, performing the duties of a truck driver, including picking up and dropping off containers. At the time of the accident, he was performing his regular assigned task of picking up a container for transport to another location. Employee #1 was walking in close proximity to the truck involved in the accident. Interviews are still pending at this time. Pending further investigation, the Division had not determined if any citations would be issued. The investigation concluded that for unknown reasons at this time, an employee of Fox Transportation struck and killed Employee #1 while he backed up his truck.
