Employee Amputates Fingertip in Press

On July 1, 2012, Employee #1 was cleaning a press that manufactured potato chips. The press head measured approximately 6 in. by 9 in. in dimensions. He used cleaning tools with safety extension that were between 9 to 12 in. in length. After the machine's power was turned off at the local switch, the tools were then used. However, the main power box was not locked out, so the machinery was capable of movement. While cleaning the press, the machine unexpectedly cycled and amputated his left index fingertip. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
