Employee's Fingertips Are Amputated When Motor Activates

At approximately 12:00 a.m. on November 8, 2011, Employee #1 and a coworker were troubleshooting Rolling Unit PC-03, a unit which cuts piping at a designated length. The unit was not cutting through the piping, so they tried changing the cutting knife of the machine. This did not solve the problem, so Employee #1 removed the bottom rear panel of the cutting machine to access and observe the machine's motor. As he observed the motor, he noted that the machine's timing belt was loose. Employee #1 attempted to tighten/adjust the timing belt by placing his hands inside the machine, and tightening a bolt on the motor which adjusted the tension on the belt. As he adjusted the tension, the motor activated, turned the belt and caught the glove of his right hand between the motor's timing belt and the motor's belt pulley, cutting the four fingers of his right hand. Employee #1 pulled his right hand free from the glove and had an assistant drive him to Little Company of Mary in Torrance, CA, where he underwent surgery that included the amputation of the tips of all four fingers of his right hand.
