Investigation Summary
At approximately 5:00 p.m. on November 17, 2010, Employee #1 was working in a warehouse, when he was struck in the head by a number of small boxes and sustained a concussion. The boxes fell from a pallet located at the height of approximately 18 ft. Emergency medical personnel were summoned. They arrived and transported him to a medical center. At the medical center, Employee #1 received care and was then hospitalized.
