Investigation Summary
At approximately 6:00 a.m. on April 30, 2010, Employee #1 and a coworker were cutting a 36-inch by 36-inch pottery mold into two pieces. To cut the Pottery mold into two pieces, the employees used a 10-inch blade Skil table Saw. The coworker pulled the pottery mold through the 10-inch blade as Employee #1 pushed the pottery mold through the blade. As Employee #1 pushed the pottery mold through the saw's running blade, his right sweater sleeve got caught on the running saw blade, pulling his right arm toward the running blade and cutting his right forearm. Supervision of plant called 911, who dispatched Long Beach Fire Department. Long Beach Fire Department treated the wound and transported Employee #1 to St. Mary's Hospital. At St. Mary's emergency room, the cut to Employee #1 right forearm was stapled closed and Employee #1 was released. The accident was reported to the Division of Occupational Safety and Health Torrance Office by the Long Beach Fire Department at approximately 7:00 a.m. on the April 30, 2010. The Division arrived at Jacobson Plastic Inc on the April 26, 2010 to conduct an investigation of the incident.
