Employee's Finger Tip Is Crushed When Load Slips

At approximately 11:00 a.m. on September 9, 2009, Employee #1 was a forklift operator for a defense sub-contractor that manufactures commercial and military aircraft parts and components. He had transported a die that weighed approximately 250 lbs and measured 3 ft by 4 ft by 1 ft high. He tilted the forks downward to place the die on a pallet. He got off the forklift to position the pallet correctly beneath the forks. As Employee #1 was positioning the pallet, the die slid off the forks and crushed the tip of his left finger. He was hospitalized for treatment and the amputation of his finger tip.
