Investigation Summary
On July 2, 2009, Employee #1 was working in a storage yard, when a bulk stack of scaffolding cross braces gave way. The cross braces fell onto the employee, trapping him beneath the weight of the load. Emergency medical services were contacted. They arrived and removed Employee #1 from under the load. He was transported to Renown Medical Center, where he died four days later due to complications from his unspecified injuries.
