Employee's Foot Is Amputated When Struck by Forklift

On July 7, 2010, Employee #1, of Pace Industries Grafton Division, was directing Employee #2 into position to remove a furnace. Employee #3 was operating a Cat Propane forklift (Model Number: C60001, Serial Number: AT83F10150) heading north in the aisle way and cut the corner, striking the Employee #1 in the right shoulder pushing him to the ground and then running over his foot with the rear tire. He was hospitalized and his foot was amputated. Employee #2 observed Employee #1 being struck by the forklift and quickly responded to the incident. The incident occurred at the corner aisle in the furnace/melting area. The location of Employee #2 at the end of the aisle was approximately 17 ft (west to east). The width of the aisle Employee #3 was operating the forklift was approximately 10 ft (west to east). The distance from the rows to the corner of the aisle was approximately 27 to 28 ft (south to north). Employee #1 was said to be approximately 5 to 6 ft south of the aisle and approximately 1 ft east of the aisle.
