Investigation Summary

Investigation Nr: 202634234
Event: 05/25/2011
Employee Is Injured When Caught Between Pole and Forklift

At approximately 6:30 a.m., on May 25, 2011, an employee was operating a Crown, RC Number 651 stand up type fork lift (Model NumberRC3020-35, Serial Number1A241628) in the warehousing facility. He was traveling in a westerly main aisle way and looking at the paper work to pick up orders from FedEx area. He was driving in reverse direction and noticed that the fork lift came too close to vertical metal support column Number G3 (dimensions, 9.5-in. by 9.5-in.) and stuck his left leg outside the lift cab. At that time, the fork lift impacted the column pinning his left lower leg between it and the frame of the fork lift causing serious injury. He sustained serious left leg foot degloving injuries. The employee was taken to the Arrowhead Regional Medical Center, Colton, CA for treatment. The employees left leg was amputated below knee and he was hospitalized for more than eight days.
