Employee Seriously Injured in Paper Roll Accident

At approximately 12:30 p.m. on April 26, 2011, Employee #1 and a coworker were operating a paper rewinding machine, of a manufacturing facility. During rewinding a roll, Employee #1 noticed that some damaged paper was being rewound. Employee #1 asked the coworker to stop the machine to save the roll. The front guard was in the up position, and Employee #1 cut a layer of paper from the roll bottom side, by using a wooden broom stick to prepare for splicing the paper. After cutting the paper, Employee #1 walked toward the right side of the machine when the operator opened the side latches and removed nuts of the roll shaft mandrill ends, without telling him. As the roll started to spin, it became loose and fell out of the machine pinning Employee #1 between it and the fork lift mast, injuring him seriously.
