Employee Is Injured While Clearing Jammed Orange Bin

At approximately 3:30 p.m. on April 4, 2011, Employee #1, a bin dump operator, was clearing a jammed orange bin that weighed approximately 1,000 lbs. Employee #1 did not deenergize the chain conveyor or apply lockout procedures. Employee #1 was caught between the orange bin unstacker frame and a column of three bins filled with oranges. Employee #1 sustained multiple unspecified bodily injuries and was hospitalized.
