Investigation Summary
At approximately 2:00 p.m. on December 21, 2010, Employee #1, a temporary employee, was assisting the operator of a UTECO printing press at Station #6. The central impression roll of the printing press needed to be cleaned. The press operator asked Employee #1 to go to the other side of the press, where a ladder was located. The press operator began to move the roll in inching mode. As Employee #1 climbed up the ladder, the right arm of his sweat shirt was caught in the moving gears. Employee #1 suffered a deep laceration to his right arm and was hospitalized. (Note: Narrative describes printing press as a UTECO, Annodi Costruzionr 1993, Maticola# 996, Tipo-Ruby RR609. An internet search for this printing press was negative.)
