Employee's Finger Is Amputated When Caught by Machine Jaws

At approximately 8:00 a.m. on October 05, 2010, Employee #1, who had been a regular, fulltime employee for 15 years, was working for his employer, a nonunionized snack manufacturer that employed 30,000 employees. Using his right hand, Employee #1 was trying to clear a jammed plastic film from the jaws of a bag-making machine (Woodman Polaris Bag Maker; Model/Serial Number PLBA125LN0534691). The jaws closed, and his right middle finger was caught between the jaws and amputated. Employee #1 required surgery.
