Investigation Summary
At approximately 8:41 a.m. on August 19, 2010, Employee #1 suffered fatal injuries when he was run over by the right rear tire of a dump truck, at the demolition site of a concrete reservoir building. After the truck was loaded with concrete rocks, Employee #1 removed the fallen rocks from underneath the trailer, then motioned to the coworker to go. The coworker went into the cab of the truck. Around this time,Employee #1 happened to be under the right rear side of the trailer for unknown reason, at which time, the coworker operated the truck in the forward direction running over Employee #1 under the right rear tire resulting in his death.
