Employee Is Injured in Fall from Elevation

At approximately 10:50 a.m. on July 9, 2010, Employee #1, the rock and rope climbing director, a ropes course instructor, and two coworkers were setting up a zip line for campers. A belay pulley (SRD) was need for the zip tree, but it was entangled in the staples on the climbing wall post. The instructor tried to untangle the cord by maneuvering it from the ground, but was unsuccessful. To untangle the belay pulley cord, Employee #1 climbed the zip tree via an aluminum extension ladder and traveled across the climbing wall post through the use of a Black Diamond Alpine Bod climbing harness (model number CE0082, 5111 revision B with revision number 81935, and style number 650026) and crab claws. She untangled the pulley cord. Afterwards, she began the return to the zip tree, but first unhooking the crab claws from the metal cable and then hooking them to a separate pulley located in front of the belay pulley. The pulley had a missing carabiner on the bottom side, which caught the top metal cable and orientated the pulley upside down. Employee #1 mistakenly attached the crab claws on the wrong side of the pulley. As she began to across, the pulley detached from the cable at the top via the missing carabiner end, causing her to fall approximately 30 ft, and landing on wood chips. She suffered serious bodily injuries and shattered left wrist. Emergency medical personnel were summoned and transported her to a medical center where she received treatment and was hospitalized.
