Investigation Summary
At approximately 1:30 p.m. on July 3, 2010, Employee #1, a machine operator, was sanding the still running rolls of a machine. The sand paper was being held by a pair of pliers when the rolls pulled his right index finger into the pinch point fully amputating it. Employee #1 was initially transported to the occupational clinic, then transferred to a community hospital and finally to Loma Linda University Medical Center, where he was hospitalized.
