Investigation Summary
At approximately 10:15 AM on September 24, 2009, Employee #1, a carpenter, was operating a portable rotating Skillsaw, cutting a piece of wood approximately 8 inches in length from a piece of 1 inch thick plywood 3 inches x 8 feet. During the cut, the tip of the employee's index finger was amputated. The employee was transported by his Supervisor to Valley Medical Center in San Jose by the superintendent. Employer reported the accident to Cal/OSHA on September 24, 2009 via phone at approximately 2:15 pm. The investigation into the accident began on October 6, 2009 at approximately 10:00 am. Opening conference was held with the Superintendent and General Superintendent. The Division was escorted to the scene, took photographs and measurements. At the time of this investigation, the scene had been changed. The Skillsaw was moved by the injured employee.
