Employee Is Killed in Fall from Forklift Basket

At approximately 4:30 p.m. on December 28, 2009, Employee #1 and coworkers were using a rented all terrain forklift, with a job made ply wood box to load and remove trash and debris from the garage. The box consisted of three sides and a bottom. The box was secured by a 4-in.wide nylon strap that was looped around the forklift backstop and nailed to the sides of the box. The forklift operator was not trained to use this equipment. At the end of the day, Employee #1 and two coworkers began to remove icicles from the inside corner of a building at roof level. Employee #1 got into the plywood box and was raised to the roof where he used a hammer to knock down icicles. Employee #1 was working on the far right side of the box and leaned out toward the corner of the building to remove icicles. The box became unstable, tipped, and Employee #1 fell and was killed.
