Employee Gets Arm Crushed While Operating a Forklift

At approximately 6:30 a.m. on January 15, 2013, Employee #1, a forklift operator of Sun Pacific was seriously injured when his arm was crushed between a control panel door and the roll bars on a Toyota forklift (Model: 7FBCU25; Serial: 71943) that he was operating. Employee #1 was attempting to operate the conveyor machine to move bins. The outer lever on the control panel used to turn the controls on was not working so Employee #1 opened the panel to access the inside switch while still on his forklift. Inside the panel there are live parts at the switch and relays. Employee #1 accidentally made contact with one of these live parts and was shocked which caused him to accidentally accelerate the forklift and crush his arm between roll bars and the open panel door. Employee #1 was in the hospital five days because of the crushing injury to his upper right arm.
