Employee Loses Fingers in Accident Involing a Kettle

Employee #1 was emptying out the contents to a kettle. Initially, contents were removed from the top of the kettle using a bucket. Later on, the contents were emptied from the bottom of the kettle by activating a valve to allow the contents to flow downward. Employee #1 was monitoring the flow of the contents and was giving instructions to a coworker to open and close the valve. Unfortunately, there was miscommunication between the two of them, as coworker closed the valve while Employee #1's fingers were still inside the pipe. As a result of the accident, three of Employee #1's right fingers were amputated. Employee #1 was hospitalized following the incident.
