Investigation Summary
At approximately 7:42 a.m. on February 17, 2010, an employee was erecting a Perry Scaffold. The employee finished the first level platform and was placing one of the side railing or end frame access ladder. The end access ladder was not inserting easily to the base, so the employee proceeded to climb the access ladder up to the fourth rung. He was balancing himself by pushing his hands against the air duct. He then stomped his right foot onto the end frame. During the process of stomping his foot, he lost his balance and then tried to grab onto the straps on the air duct. The employee fell backward and severely lacerated his wrist. The employee spent days (not specified) in the hospital at Los Angeles County USC, where he received surgery for his wrist. Employee received scaffold training from Union trade school. In addition, the employee stated that he knew the fall protection policy for the job site and which fall protection required at six feet.
