Employee Fractures Ankle in Fall from Rack

At approximately 3:20 p.m. on December 23, 2009, Employee #1 was performing duties of a forklift operator, which included retrieving orders of food from a rack. The facility was involved in cold storage. A nonsupervisory coworker asked him for help to pick out an order of his own. The coworker had Employee #1 climbed onto a forklift, which was against company procedures, and elevated him to the rack at an approximate height of 14.75 ft. He retrieved the order and placed it onto the pallet of the forklift. As Employee #1 was maneuvering the order onto the pallet, he lost his footing and slipped off the rack. Employee #1 landed on his feet first, fracturing his right ankle. He was transported to a medical center, where he received treatment and was hospitalized.
